Care Plan creation 

 

When Service Providers first on-board a new Participant, a key consideration is the building of a holistic and simple to implement Care Plan. This key document informs Support Workers of what they must know prior to commencing work with the Participant, and provides relevant and need-to-know information in order to successfully work with a Participant as part of a team.

  1. We take care of the building of the Care Plan: After signing a confidentiality agreements and sharing of information, you upload documents to our secure document upload.

 

  1. You use the Participant Name & Organisation Name OR Unique ID and Unique Org ID and upload documents: Care Plan Document Upload

 

  1. We Schedule a Google Meeting with key persons: Once we receive your Participant documents, we’ll reach out to schedule a friendly chat. This can be over the phone, via video call, or in person – whatever suits you best. It’s a chance for us to meet, discuss your needs, and clarify the range of the Care Plan depth. 

4. Care Plan Development: Based on our conversation and your supplied documents, we’ll work together to develop a personalised Care Plan. This plan will outline how we can best assist the Participant with a comprehensive initial Care Plan. This plan is delivered within 1 week of documents being supplied.

 

5. Monitoring and Continuous Development: Once the initial Care Plan is developed and approved, we can support the continued development of the Care Plan over time, so that key updates and monitoring is occurring, based on the needs of the Organisation.
